Perimenopause and Mental Health: Understanding the Intersection
When mood and stress tolerance shift, the explanation is often more than psychological.
Perimenopause is the transitional period preceding menopause, during which hormonal fluctuation is common. Many women report changes in mood, sleep, concentration, and stress tolerance during this time. This article is educational and is not medical advice.
Commonly reported experiences
- Mood changes, irritability, or heightened anxiety
- Disrupted sleep, including early waking
- Difficulty with concentration or word retrieval
- Reduced tolerance for stressors previously managed easily
- Changes in energy and motivation
What the research does — and does not — say
It would be too simple to say that hormones cause anxiety. A systematic review examining whether the menopausal transition elevates risk for depression and anxiety found that outcomes appear to reflect an interaction of hormonal change, vasomotor symptoms, sleep disruption, prior psychiatric history, and stressful life circumstances rather than any single cause (Alblooshi et al., 2023).
Clinically, that matters. It means the honest question is not "is this hormonal or is this stress" but "what is all of this together, and what does each part need?"
Why it is frequently missed
Because the symptoms are non-specific, and because they arrive in a season already full of plausible explanations. Women are often told they are simply stressed — which may be true and still incomplete.
